    • You will find registration forms on this website, but they will not suffice for matriculated students; if you are registering after the registration deadline, you will need to contact the Warner School Registrar to receive the appropriate hard-copy forms to complete and submit.

PLEASE NOTE: All forms may now be sent directly to the Tuition Benefits Office (44 Celebration Dr, Suite 2300, P.O. Box 270453, Rochester, NY 14627-0453) without Registrar Certification. Be sure to keep a copy for your records. (Should you need to fax the forms, the number is 585.273.1054.)
